They known Bruno, and Bruno can tell when she's hearing a snarky comment from them, which Dana imediately follows up with an explanation, that Samantha is implying there is an inflamed part of his brain causing misfiring, which is really just a topic he's interested in which she is not. :) I think the main answer is inflection.
